Risk Management Evolves

New doctrine on risk management has emerged, but industry/government cooperation and actual training need to improve.

All too often, the NTSB and the FAA have failed to adequately explain the root causes of most fatal general aviation accidents. Typically, the final accident reports describe the event that causes the crash, such as “loss of control,” but fail to sufficiently probe for the deeper cause(s) that led to the accident. As one result, those reports and our preventive measures usually focus on physical skill improvements and other remedies that often don’t improve general aviation’s stagnant fatal accident rate. The good news is industry recognizes the problems. The bad news is it’s having trouble with implementation.
