At approximately 1045 Central time, the airplane was substantially damaged upon impact with terrain following loss of engine power on initial takeoff. Both the commercial pilot and passenger were fatally injured. Visual conditions prevailed.

According to initial statements provided by witnesses, the airplanes engine sounded normal until it was 100 to 200 feet agl. At this point the airplanes engine quit, and dark smoke emanated from the airplane. The airplane began a left, 30-degree-bank turn. The engine sounded as though it was attempting to restart prior to the airplane colliding with terrain.
