At about 1900 Mountain time, the airplane impacted terrain shortly after takeoff. The flight instructor was seriously injured and the private pilot receiving instruction sustained minor injuries. The airplane was substantially damaged. Visual conditions prevailed.

Shortly after takeoff, at about 50 feet agl and approximately 90 KIAS, the airplane started to bank to the right. Despite control inputs from both the private pilot and the instructor, the airplane continued to the right and impacted terrain about 30 feet perpendicular to the runway.  This was the first flight after the airplane had undergone an annual maintenance inspection approximately one month prior.
