The airplane was substantially damaged when it impacted terrain at about 0950 Eastern time during takeoff from a turf runway. The private pilot and a passenger were seriously injured. Visual conditions prevailed.

The pilot subsequently reported the airplanes nose gear began to collapse during the takeoff roll. The airplane became airborne briefly, before it impacted the ground and struck trees. An FAA inspector noted a series of propeller strikes approximately 1440 feet from the beginning of the runway.
